community Aging in Place--
Advancing Better Living for Elders
Keeping seniors living safely and independently in their own homes
CAPABLE, was developed by Johns Hopkins University School of Nursing. The program’s goal is to allow older adults to remain in their homes longer, achieve better health outcomes, and decrease medical cost. CAPABLE consists of about 10 home visits over 4-5 months from a registered nurse, occupational therapist, and home repair service. As part of the program, a handy worker will make minor home modifications or install assistive equipment to help improve the safety and functionality of the home.
We are the first CAPABLE program in Alaska, and thanks to a grant from the State of Alaska Department of Health and Social Services, the program is completely FREE!
Patients set their goals—and then we help them meet them
Participants can identify any type of goal they’d like to address, such as:
Doing laundry, showering, housework, meal preparation, taking their medications, getting dressed and undressed, or reducing fall risk

- 65 years or older
- Have at least some difficulty with at least one every day activity (bathing, dressing, grooming, cooking, etc)
- Not terminally ill or receiving active cancer treatment
- No Alzheimer’s or Dementia
